As stated, all information is confidential and will only be shared with essential teachers or upper management of Multiply Music Group Ltd, who will need to be made aware of this information when teaching said child/adult. 

It’s the policy of Multiply Music Group Ltd that no employee or customer will receive less favourable treatment on the grounds of martial status, gender, race, age, nationality, religious beliefs, sexual orientation or disability. 

This document is to be completed by the parent / guardian of the child in question. Please note that if you don’t state that you wish to allow Multiply Music staff members to perform first aid if needed, your child will not receive care.
